Forms

Applications and sign-up forms, filled in inside Discord

Applications, ban appeals, and sign-up forms your members fill in right inside Discord — you write the questions, and submissions land wherever you route them for your staff to accept or reject.

Turning it on or off

Every server chooses for itself — switch Forms on or off from your dashboard, under Messages & Automation. When it's off: members cannot open or submit your forms.

Setting it up

  1. 1

    Write your questions

Pick the right type for each — a select menu beats free text whenever the answer is really one of a fixed set.

  2. 2

    Add validation where it matters

A regex or length check catches malformed answers before a human has to notice and reject them manually.

  3. 3

    Use conditional questions to branch

Instead of building two nearly-identical forms for two different applicant types.

  4. 4

    Set where submissions go

And decide who has permission to approve or deny them.

Question types

Short text, a full paragraph, a select menu of fixed choices, a file/image attachment, or a live picker for a role, user, or channel — seven types covering most application and intake needs without ever needing free text where a picker would be more precise.

Validation and conditional questions

Any text question can enforce a regex pattern, a min/max length, or restrict it to a number within a range. Conditional questions only appear once every condition you've set on them matches an earlier answer — so a form can branch ("only ask about your Minecraft username if they selected Minecraft above") without needing separate forms for each path.

Reviewing submissions

Submissions carry a lifecycle — pending, then approved or denied by your staff — and the decision can trigger a follow-up automatically, like an Automations rule reacting to a form_submitted trigger with the approved/denied decision as a filter.

Commands

Tips & things people get wrong

  • A select-menu question tops out at 25 choices — Discord's own limit on that component, not something Hez can raise.
  • Conditional questions are AND, not OR — a question with two showIf conditions needs both to match before it appears.
